The Merit-based Incentive Payment System (MIPS) Value Pathways (MVPs) registration window is open for the 2026 performance year. Individuals, groups, subgroups, and Alternative Payment Model (APM) Entities that wish to report an MVP can register until November 30, 2026, at 8 p.m. ET.
To register, you’ll sign in to the QPP website with your HCQIS Access and Roles Profile (HARP) account.
You must have a HARP account and a QPP Security Official role to complete the MVP Registration.
For more information on HARP accounts, please refer to the Register for a HARP Account document in the QPP Access User Guide (ZIP, 5MB).
For more information on obtaining the QPP Security Official role, review the Connect to an Organization document in the QPP Access User Guide (ZIP, 5MB).
Prepare for MVP Registration
Before you register, you’ll need to have the following items identified:
The MVP you plan to report
Whether you plan to administer the Consumer Assessment of Healthcare Providers and Systems (CAHPS) for MIPS Survey, if it’s a quality measure option in your selected MVP
Whether you want to be evaluated on an outcomes-based administrative claims quality measure, if it’s a quality measure option in your selected MVP
The participation option you plan to use individual, group, subgroup, or APM Entity
Additional Information for Group Registration
Starting in 2026, to register for MVP reporting as a group, your practice will need to attest to their specialty composition (whether you’re a single specialty group or multispecialty small practice) during the MVP registration process. We won't make this determination for you. All other groups will need to participate as subgroups or as individuals (if eligible) to report an MVP.
A single specialty group means a group that consists of clinicians in one specialty type, or clinicians in more than one specialty type with a single clinical focus of care. For example, a group consisting of internists, cardiologists, nurse practitioners, and physician assistants that are focused on providing cardiovascular care could attest to being a single specialty group and register to report an MVP as a group.
How to Register
Individuals, groups, subgroups and APM Entities will register on the QPP website. You'll need to have the Security Official role to register your organization. Please refer to the QPP Access User Guide (ZIP, 5MB) for information about obtaining a Security Official role for your organization. To register: Sign in to QPP

Note: If the CAHPS for MIPS Survey is included in your chosen MVP and you plan to use it as one of your four required measures, you must complete both your MVP registration and a separate CAHPS for MIPS Survey registration by June 30, 2026, at 8 p.m. ET. Visit the QPP website for details on registering for the CAHPS for MIPS Survey.
Registration is open through June 30, 2026, at 8 p.m. ET for the Consumer Assessment of Healthcare Providers and Systems (CAHPS) for the Merit-based Incentive Payment System (MIPS) Survey for the 2026 performance year.
APM Performance Pathway (APP) or APP Plus: Groups and APM Entities (excluding Medicare Shared Savings Program Accountable Care Organizations (ACOs)) with 2 or more clinicians that choose to report the APM APP or APP Plus must register to administer the survey;
MIPS Value Pathway (MVP): Groups, subgroups and APM Entities with 2 or more clinicians reporting a MVP can register to administer the survey.
Who doesn’t need to register?
Medicare Shared Savings Program ACOs
These ACOs are required to report the APP Plus so they’re automatically registered for the CAHPS for MIPS Survey, which is a required measure in the APP Plus.
These ACOs don’t need to register but will still need to contract with a CMS-approved vendor to administer the CAHPS for MIPS Survey. Instructions will be provided later.
Who must register?
Groups and APM Entities (excluding Shared Savings Program ACOs) with 2 or more clinicians (including at least one MIPS eligible clinician) that choose to report the APP or APP Plus must register because the CAHPS for MIPS Survey is a required measure when reporting the APP or APP Plus.
Groups, virtual groups, and APM Entities with 2 or more clinicians (including at least one MIPS eligible clinician) that intend to administer the CAHPS for MIPS Survey as 1 of their 6 quality measures for reporting traditional MIPS.
Groups, subgroups, and APM Entities with 2 or more clinicians (including at least one MIPS eligible clinician) that are registered to report the following MVPs and intend to administer the CAHPS for MIPS Survey as 1 of their 4 quality measures:Adopting Best Practices and Promoting Patient Safety within Emergency Medicine MVP, Advancing Cancer Care MVP, Optimal Car for Patients with Urologic Conditions MVP, Value in Primary Care MVP, and Vascular Surgery MVP.
How to Register
Groups, virtual groups, and APM Entities will register on the QPP website. You'll need to have the Security Official role to register your organization. Please refer to the QPP Access User Guide (ZIP, 5MB) for information about obtaining a Security Official role for your organization. To register:
Subgroups interested in administering the CAHPS for MIPS Survey as 1 of their 4 required quality measures will need to complete their MVP registration prior to registering for the CAHPS for MIPS Survey. After completing the subgroup’s MVP registration, they will need to contact the QPP Service Center by emailing QPP@cms.hhs.gov, creating a QPP Service Center ticket, or calling 1-866-288-8292 (Monday through Friday, 8 a.m. – 8 p.m. ET) to register for the CAHPS for MIPS Survey.
Today, the Centers for Medicare & Medicaid Services (CMS) released the Quality Payment Program (QPP) final participation and performance data results for the 2024 performance year (PY), which includes both Merit-based Incentive Payment System (MIPS) and Advanced Alternative Payment Model (APM).
We’ve streamlined the number of available resources to provide interested parties with the most meaningful and useful data. Aggregated data in the Results At-A-Glance answers the most frequently asked questions we receive, while the clinician-level data in the PUF can be aggregated to evaluate metrics that are most important to you.
Data Highlights
The 2024 QPP Participation and Performance Results At-a-Glance includes:
The total number of MIPS eligible clinicians that will receive a 2026 MIPS payment adjustment, including the number and percentage of non-reporting clinicians who didn’t submit any data.
The mean and median 2024 MIPS final scores, as well as the unweighted mean and median performance category scores.
The maximum 2026 MIPS payment adjustment percentage (applied to clinicians with a final score of 100 points).
The total number of Advanced APM Participants and Qualifying APM Participants (QPs).
The 2024 QPP PUF includes:
The 2024 MIPS final score and 2026 MIPS payment adjustment percentage applied to each MIPS eligible clinician.
Indicators of the participation option (e.g., individual or group) and MIPS reporting option (traditional MIPS, MIPS Value Pathways (MVPs), or APM Performance Pathway) associated with the clinician’s final score.
The performance category scores and associated weights attributed to each MIPS eligible clinician’s final score.
The measures, activities, and their associated achievement points which contributed to each MIPS eligible clinician’s performance category score, and ultimately final score.
Additional Resources
To learn more about the 2024 QPP data resources listed above, you can review the following supporting documentation:
In response to the Hawaii Kona Low Weather Systems, and the Commonwealth of the Northern Mariana Islands Super Typhoon Sinlaku, as identified by both the Health and Human Services (HHS) Public Health Emergency (PHE) declaration (PHE | Hawaii - Severe Storms; PHE | Northern Mariana Islands – Super Typhoon Sinlaku) and Federal Emergency Management Agency (FEMA) disaster declaration (DR-4909-HI; DR-4910-MP), the Centers for Medicare & Medicaid Services (CMS) has determined that the MIPS automatic EUC policy will apply to MIPS eligible clinicians in the designated affected counties of Hawaii and municipalities of the Commonwealth of the Northern Mariana Islands for the 2026 performance year.
MIPS eligible clinicians in these designated areas will automatically qualify and will have all 4 performance categories reweighted to 0% during the data submission period for the 2026 performance year (January 4, 2027 to March 31, 2027). This will result in a score equal to the performance threshold, and they'll receive a neutral payment adjustment in the 2028 MIPS payment year.
However, if MIPS eligible clinicians in these designated areas submit data on 2 or more performance categories, they’ll be scored on those performance categories and receive a 2028 MIPS payment adjustment based on their 2026 MIPS final score.
NOTE: The MIPS automatic EUC policy doesn’t apply to MIPS eligible clinicians participating in MIPS as a group, subgroup, virtual group, or Alternative Payment Model (APM) Entity. However, groups, virtual groups, and APM Entities can request reweighting through the EUC Exception application. Subgroups will inherit any reweighting approved for their affiliated group; they can’t request reweighting independent of their affiliated group’s status.
