Chemical company fined after worker dies from fall
A chemical logistics company has been fined £425,000 following the death of a warehouse supervisor who fell from a mobile loading ramp.
HSE’s investigation found:
- the company failed to access risks with pedestrians using the mobile loading ramps
- workers were permitted to access the ramps without handrails
- the company had not considered practicable alternatives to avoid work at height

Company fined and director handed suspended sentence after death of apprentice
The apprentice joiner was working alone at a property undergoing renovation, when a stack of wooden board material fell on her, causing fatal injuries.
HSE’s investigation found the company had:
- failed to provide safe systems of work, adequate information, instruction, training and supervision
- no suitable lone working policy or procedure
- no suitable policy or procedure for the storage of board materials
- an inadequate risk assessment prior to the incident
The investigation also found that these failures were attributable to the neglect of a director. The company was fined £50,000 and the director was sentenced to 26 weeks imprisonment, suspended for 2 years.

Academy trust fined after worker falls from roof
A site supervisor suffered life-changing injuries including bleeding on the brain and fractures to their face, hand and foot after falling from the roof of an Academy trust.
HSE’s investigation found:
- the roof had no edge protection or other means to prevent a fall from height
- there was no standalone work at height risk assessment or safe system of work
