We have created a group page for parents, athletes, and coaches to receive Activities Office updates. You can access information via web or app.

Athletic Registration
Our athletic registration site, FinalForms, is now live! You should have received an email to register your athlete for sports. Please check your inbox (spam included) to complete the athlete registration. For rising 9th graders, your last middle school may not be populated yet. Please type “Not Listed” if your previous school isn’t listed. This is mandatory to participate in all VHSL sports.
Online Athletic & Activity Registration (FinalForms) must be completed each school year
- FCPS will be using a new athletic registration platform, FinalForms, beginning Fall 2026.
- All parents with students registered in grades 9-12 will receive a confirmation email from FinalForms. These emails will be delivered to the email address associated with the enrolling parent. If you have not received this, please go to https://fairfax-va.finalforms.com/
- After you receive the initial email from FinalForms, view the Parent Playbook (linked below) to get started in FinalForms.
VHSL Physicals
We are accepting physicals all summer. The activities office is open Monday-Friday, 9-2pm. Parents are also able to upload physicals directly into FinalForms.
New!!!! Wristband Eligibility System
To streamline our tryout process, we are implementing a new athlete check-in system using wristbands.
Before an athlete can participate in tryouts, they must check in at the Activities Office to receive their wristband. To be eligible for a wristband, athletes must meet the following two requirements:
- Have a valid VHSL physical form on file (can be uploaded in FinalForms, but can be turned into our office)
- Complete the online athletic registration via FinalForms.

Brandon Millias- Boys Volleyball, Head Coach
Coach Millias joins us with a strong background and a wealth of coaching experience across Northern Virginia and New York. Most recently, Brandon served as the Varsity Assistant Coach at Robinson Secondary School, and we are thrilled to bring that local competitive experience and his passion for student-athlete development to South Lakes.
He also holds a degree in Physical Education and a minor in Athletic Coaching and is dedicated to building a high-energy environment where players are challenged to grow both as individuals and as a cohesive unit.

Adam Temple – Swim & Dive, Head Coach
Coach Temple is a familiar face to our students, as he currently serves as an English teacher right here at South Lakes. He is a proud South Lakes alumnus, graduating in the class of 2001. During his time as a student-athlete here, he both swam and played football, before going on to play football at Emory & Henry College.
Coach Temple has experience and a deep passion for both swimming and coaching young athletes. In addition to his background in education, he has honed his coaching skills at the club level, previously coaching for the Herndon Aquatic Club.
Coach Temple is excited to step into this role and is looking forward to working with our student-athletes to continue to build on our program's strong traditions both in the classroom and in the pool.

Alexa Phillips – Dance, Head Coach
Please welcome our new dance coach, Alexa Phillips, to South Lakes! Coach Phillips has an amazing background. She has danced all over the world with Royal Caribbean Cruise Line and even represented Team USA at an international dance competition in Poland. On top of her incredible professional experience, she has been teaching and choreographing locally, at Studio Bleu and her current role teaching at Mission Dance Project. She is talented, professional, and versatile, and we are so lucky to have her leading the Seahawks dance team this season!

Spencer Alston- Boys Lacrosse, Head Coach
We are pleased to announce Coach Spencer Alston as our new Boys Lacrosse Head Coach!
Coach Alston is a familiar face at South Lakes. He served as our Varsity Assistant Coach last season and currently leads our Girls Flag Football program.
A 2018 South Lakes graduate and 2024 Yale University graduate, Coach Alston has a strong championship history as a Seahawk. He was a member of the football team that won two District Championships and a leader on the lacrosse team that captured three consecutive District Championships from 2016–2018 and made a State Tournament appearance in 2016.
We are excited to welcome Coach Alston to this new role and look forward to his leadership this spring!

Registration & VHSL Physicals
Activities Registration Site
We are excited to announce that Fairfax County Public Schools has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic forms for your students. FCPS and FinalForms have worked to ensure we safeguard your most sensitive information. FinalForms meets all of FCPS’ rigorous data protection requirements and will be a tremendous new partner.
The most exciting news is that FinalForms saves data year-to-year, meaning that you will never need to enter the same information twice! FinalForms will also pre-populate information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
All parents with students registered in grades 9-12 will receive a confirmation email from FinalForms. These emails will be delivered to the email address associated with the enrolling parent. Please check your spam/junk folder and if the FinalForms email landed there please “mark as safe” emails from FinalForms and add them to your “contacts”. Your school, and the Office of Student Activities and Athletics, will use FinalForms to communicate directly with the enrolling parent quite regularly throughout the school year.
After you receive the initial email from FinalForms, click on the Parent Playbook (below) to get started in FinalForms. If you require any support during the process, scroll to the page bottom-right and click “Get Help”.

Physical Process
Legislation introduced in the Virginia General Assembly and recently signed into law will result in two significant changes to the athletic pre-participation examination (physical) requirements for FCPS student-athletes:
- Beginning July 1, 2025, athletic physicals will be considered valid for a period of 14 months from the date the form is signed by the medical provider completing the exam. This is the date on page 3 of the VHSL Athletic Participation/Parent Consent/Physical Examination Form (revised January 2021).
- Beginning May 1, 2025, FCPS student-athletes are EXPECTED to have athletic physicals completed on the new VHSL Athletic Participation/Parental Consent/Evaluation Form (revised April 2025)
- The new form has been updated on the website
Transfer Students
ALL TRANSFER STUDENTS by default are INELIGIBLE. More information can be found on HERE
The NEW Guardian Application Process must be started before the transfer students is considered a "bona fide" student.
NO TRANSFER IS ELIGIBLE UNTIL THERE IS NOTICE FROM THE GUARDIAN SYSTEM THAT INCLUDES A VHSL CITATION AND POST SEASON ELIGIBILITY.
