The Centers for Medicare and Medicaid Services (CMS) released new 2025 Medicaid and Children’s Health Insurance Program (CHIP) Child and Adult Core Set data and products plus new external quality review summary tables with performance-related information about states’ Medicaid and CHIP managed care programs for 2025-2026.
The Child and Adult Core Sets of health care quality measures represent the quality of care delivered to millions of people enrolled in Medicaid and CHIP. CMS publicly releases data on measures that are reported by at least 25 states and that meet data quality standards. This release represents the most robust year of Medicaid and CHIP quality measure reporting to date: all 26 of Child Core Set measures and 34 of 38 Adult Core Set measures.
The Core Set data products posted on Medicaid.gov include:
- Core Set Data Dashboard, which features dynamic displays of performance data on 2025 Child and Adult Core Set measures. It shows detailed information, including state-specific performance, national medians, and data across time for each publicly reported Child and Adult Core Set measure.

2025 Core Set Fact Sheet, which provides an overview of state reporting, highlights measures with high and low national median performance, and identifies trends in select measures over the last three years.

Trendability Methods Brief, which summarizes the criteria CMS uses to assess trends in state performance and to identify which measures can be used to report trends for the three-year period from 2023 to 2025.

Trending Analysis of State Performance: 2023 to 2025 Child and Adult Core Sets, which provides information on trends in median state performance on measures that were publicly reported by a set of at least 20 states from 2023 to 2025 and did not have substantial changes in their technical specifications.

2025 Core Set dataset, which provides the raw data, means, medians and state-specific notes for publicly reported measures for researchers and others to use.

CMS also released new external quality review (EQR) tables summarizing key information about Medicaid and CHIP managed care program performance measures and performance improvement project topics. The tables reflect CMS’s analysis of states’ 2025-2026 EQR annual technical reports.
