Clinicians who are unsure about their eligibility to participate in the Merit-based Incentive Payment System (MIPS) for the 2025 performance year, can check their final eligibility status using the QPP Participation Status Tool.
Clinicians and groups that are opt-in eligible will need to make an election to opt-in or voluntarily report before they can submit data. No election is required for opt-in eligible clinicians and groups that don’t want to participate in MIPS.
QPP Service Center Hold Times
The QPP Service Center projects an increase in volume through the close of the QPP data submission period on March 31, 2026, which could result in longer wait times.

Use One Method to Report Issues
Due to the anticipated increase in volume at the QPP Service Center and to minimize a backlog, please use only one method of reporting for the same issue (email, phone, or CCSQ Support Central). Note: Cases are processed in the order in which they're received, regardless of how the Service Center was contacted. Please allow time for processing.
The data submission period for PY 2025 of the Quality Payment Program (QPP) is open. Data can be submitted and updated until March 31, 2026, at 8:00 p.m. Eastern Time (ET). ACOs should review their quality data for completeness and accuracy and submit by the deadline; data cannot be updated or corrected after the submission period has closed. ACOs are encouraged to report early to allow time to ensure data is accurate and correct any errors, if needed, in advance of the deadline.
Sign in using your QPP access credentials. ACO representatives with the Security Official or Staff User role in ACO-MS can sign in to the QPP website using their ACO-MS username and password.
Submit your ACO’s data for PY 2025 or review the data reported on your behalf by a third party. ACOs cannot correct errors with your data after the submission period, so it is important to make sure the data submitted on your ACO’s behalf is accurate.
Confirming your data was submitted correctly:After submitting your ACO’s final quality data, please verify that your quality measures were correctly submitted by confirming that they appear as an “APP Plus” submission in the “Reporting Summary” section of the Reporting Overview page. If your ACO’s quality data file is not correctly tagged as an APM Performance Pathway Plus (APP Plus) submission, the system may still indicate that the file has been uploaded successfully, but your ACO’s quality data file will not show up as an APP Plus submission.
To check this within the QPP User Interface, please follow the steps below:
From the Eligibility & Reporting tab, select “View APM Entity Submissions” next to your organization’s name.
Select “Edit submission” on the APM Performance Pathway Plus (APP Plus) card — this appears under Required Reporting on an ACO’s Reporting Options page.
Scroll down the Reporting Overview page and select “View and Edit” on the Quality card.
The quality page shows a summary of your ACO’s submitted measures.
To view details, select the “down” arrow next to each measure for details, including Data Completeness.
Measures will show as those that count towards your ACO’s score and those that don’t count towards your ACO’s score if a measure was reported by more than one collection type, for example.
Note that measures that “count” may not meet data completeness — you can check data completeness by selecting the down arrow next to the measure.
Your ACO may want to confirm that:
All four of the electronic Clinical Quality Measures (eCQMs), Merit-based Incentive Payment System (MIPS) CQMs, and/or Medicare CQMs show up in the “measures that count towards your score” in the APP Plus Quality Card.
All four of the eCQMs/MIPS CQMs/Medicare CQMs meet data completeness, by clicking on the measure details in the APP Plus Quality Card.
If a measure needs to be resubmitted, please include the other measures in the data file so that you do not inadvertently delete any previously submitted quality measures.
Your ACO must re-verify the submission in the Reporting Overview page as an APP Plus measure set submission before the close of submission deadline on March 31, 2026, at 8:00 p.m. ET.
For PY 2025, Shared Savings Program ACOs must report quality data on the APP Plus quality measure set. ACOs that do not submit any quality measures to the APP Plus quality measure set will be ineligible for shared savings or will owe maximum shared losses. The data submission guidance is outlined below.
For QPP JSON files, the “entityId” must be labeled with your ACO ID, the “entityType” must be labeled as “apm”, the “category” must be labeled as “quality”, and “programName” must be labeled as “appPlus” at the measurement set level for 2025. Please note that these labels are case-sensitive. The labels must not include any punctuation; otherwise, your ACO’s data submission may be rejected.
